
The Y's Blood Pressure Program supports adults who face a heightened risk of hypertension and high blood pressure, which are major factors contributing to cardiovascular disease.
Participants will take steps to improve their overall health and well-being. In a virtual setting, a trained lifestyle coach will facilitate a small group of participants in learning about healthier eating, physical activity, and other behavioral modifications over a 12-month period.
- Free program to members.
- Weekly group informational sessions on topics including nutrition and blood pressure monitoring techniques.
- Participants are given blood pressure monitoring equipment to take home and retain for personal use.
- The instructor provides regular one-on-one check-ins with participants via phone calls, online video conferencing, or in-person meetings.
- The instructor encourages participants to measure their blood pressure on a regular basis, at least two times each month, and record their blood pressure readings.
