Did you know that Diabetes is one of the most expensive conditions to treat? This program is an outcome-based program for Diabetes prevention and self management. It is suitable for individuals who are at risk for Type 2 Diabetes, are pre-diabetic, or who have a clinical diagnosis of Type 2 Diabetes.
